you dont explain why the patch needs to be applied in the install section
does that mean orthanc has an intree copy of dcmtk?
you dont mention the conditional changes for the compiler hanndling related to this: https://nordisch.org/posts/rpm-spec-files-conditionals-and-forcing/

1) We need the patch not only in the source for building, but as well in the exported source tree as the plugins use that - and will fail with a newer dcmtk package, see boo#1218423 2) No.... 3) goes back to a discuccion with Max Lin as gcc13 is needed at least for postgres and mysql plugins to work properly
